HEAR Program Application

To complete your application, you must upload the following documents and photos. Applications will not be reviewed until all required materials are submitted.

1. Geotagged Photo(s)

   You must upload clear, geotagged photos of your existing range, cooktop, dryer, and/or electric panel if applying for a rebate to replace these items. Geotagged photos contain location data (GPS coordinates) embedded in the image file.

   How to Enable Geotagging (Smartphones):

   • iPhone: Settings → Privacy & Security → Location Services → Camera → Select “While Using the App.”
   • Android: Settings → Location → Turn on Location → App Location Permissions → Camera → Allow only while using the app

2. Proof of Income

   Upload a copy of every member in the household's 1040 or W-2 forms from the prior year, a Low-Income Heating Assistance Program (LIHEAP) award letter, or four consecutive weeks of paystubs.

3. Signed Terms and Conditions

   You must upload a completed and signed Terms and Conditions Form.

   • Download the form from the application portal.
   • Review all terms carefully.
   • Sign and date the form (electronic signatures are accepted).
   • Upload a scanned copy or clear photo of the signed document.

4. Signed Landlord Agreement Form (If Applicable)

   If you are a renter, you must upload a signed Landlord Agreement Form.

   • The property owner must review and sign the form (electronic signatures are accepted).
   • Upload a scanned copy or clear photo of the signed agreement.

Applications submitted without a required Landlord Agreement will be considered incomplete.
